What is OMC MyChart?

At OMC, our mission is the delivery of exceptional patient care focusing on caring, quality, safety, and service. This includes giving our patients the tools to help them be active partners in their healthcare. To help achieve this, we offer our patients OMC MyChart, an interactive health management tool that gives you 24/7 access to your electronic medical record. It helps you to securely manage and receive information about your health. With OMC MyChart, you can use the Internet to:

  • Manage appointments.
  • View your health summary from the OMC MyChart electronic health record.
  • Access test results.
  • Request prescription renewals.
  • Access trusted health information resources.
  • Communicate electronically and securely with your medical care team.

Is there a fee to use OMC MyChart?

There is no additional cost to sign up for and have an OMC MyChart account.

How do I sign up?

Patients who wish to participate will be issued a OMC MyChart activation code during their clinic visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may sign up by clicking on the "Sign Up Now" button, then selecting "Sign Up Online". Please allow up to 7 business days to complete sign up without an activation code.

When can I see my test results in OMC MyChart?

Your test results are released to your OMC MyChart account after your healthcare provider has reviewed them. This is generally within 1-12 days.

Since you are viewing the full, unedited results, some of the language may not be familiar to you and might even be confusing or concerning. Please discuss any questions raised by these results at your next visit. See below for more details on test results in MyChart.

  • Lab results and Imaging (excluding CT/MRI/PET/Ultrasounds): Automatically released to OMC MyChart once the result is finalized.
  • Sensitive labs and imaging: These sensitive results will be released automatically to MyChart 5 days after the result is finalized.

If some of my health information on OMC MyChart is not correct, what should I do?

Your OMC MyChart information comes directly from your electronic medical record at your healthcare provider's office. Ask your healthcare provider to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my healthcare provider, when can I expect a reply?

You will generally receive an answer within 1-3 business days. Please note that OMC MyChart should not be used for urgent situations. If you are experiencing severe symptoms and/or a medical emergency, call 911 immediately.

Can I view a family member's health record in OMC MyChart?

Yes, you can view another person’s OMC MyChart with appropriate consents. This is called proxy access and allows you to see all items that appear in another person’s OMC MyChart. OMC patients over age 18 can appoint any other adult as a proxy. Parents and legal guardians of children 12 and under also can request full proxy access. Limited proxy access is available to parents and legal guardians for their children ages 13 to 17, and allows them to see their child’s allergies, immunizations and letters. Children ages 13 to 17 can also grant full proxy access to their parent or legal guardian by completing the appropriate forms. Can I view all of my child’s health information on OMC MyChart?

Yes. You will have full access to your child's information from birth through age 12. Once your child turns 13, your access will become limited. You and your child may complete a proxy consent form to grant you full access to their information. Once your child turns 18, your access to their information will end.

Can I ask questions regarding a family member from my OMC MyChart account?

OMC MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one OMC MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own OMC MyChart account.

I forgot my password. What should I do?

Click the "Forgot password" link on the sign-in page to reset your password online. You may also contact our OMC MyChart Patient Support Line at 507.287.2780 to request a new, secure password.

Can you send me a new activation code as I have lost it, let it expire or did not receive it?

Contact us at 507.287.2780. We would be happy to send you a new code, or you can sign up without an activation code by clicking on the "Sign Up Now" button, then selecting "Sign Up Online". Please allow up to 7 business days to complete sign up without an activation code.

Where can I update my personal information (e.g., home address, e-mail or change my password)?

Log into OMC MyChart and from the left menu, go to the Preferences section and select the appropriate option.

How is OMC MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all OMC MyChart messaging is done while you are securely logged on to our website.

What is your Privacy Policy?

OMC MyChart is owned and operated by OMC MyChart and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by OMC MyChart.

I was logged out of OMC MyChart, what happened?

We aim to protect your privacy and security of your information. While logged into OMC MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of OMC MyChart. We recommend that you log out of your OMC MyChart session if you need to leave your computer for even a short period of time.

What do I need to use OMC MyChart?

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer).

You can also view MyChart from your iPhone or Droid phones. Go to Apple® App Store or the Android™ Market to download your MyChart Mobile App. See the next question for more information.

My activation code does not work, what should I do?

For your security, your activation code expires after 14 days and is no longer valid after the first time you use it. If you still have problems, call our OMC MyChart Patient Support Line at 507.287.2780.
